When I first bought it the idle temp was about 50-60. Now it's 73C. I have it on a smooth table on the Performance profile.

Vortex 3 17.3
7970M
i7-3610QM

What can I do to bring it down to 60 or even 50 again? I play games for long duration and that brings it up to like 85, so that is not good.

Your cpu should be able to stand 85 and higher. How long do you have the laptop for?

Not sure what you mean...
I have the laptop for as long as I want, it's mine.
I have had the laptop since the start of Summer (2012).
I have the laptop running pretty much most of the day, and play games ~5 hours a day.

I think that's what Keynes meant "how long do you have it on for".

You could get a cooling pad of some kind? That should help lower your temps a few degrees. Also you could consider giving the laptop a dust/general clean as there is likely some build up around the intake fans.

...but how would one go about creating a nipple pie?

Sounds a bit fiddly to me :)

Right now it's like ~90C on idle. All I have is an extended monitor, and using it alongside the laptop screen. There is plenty of room under the laptop...
What can I do to reduce this? Would you recommend checking for a way to clean the dust from the fans or something?

I'd give PCS a call mate, thats very high.

I dont work for them so no idea what they will suggest, but I'd probably go for some fresh thermal paste. Something like this - http://www.amazon.co.uk/Arctic-Silv...BPAI/ref=sr_1_3?ie=UTF8&qid=1364658519&sr=8-3
