Does this suggest they encountered a problem? Would they add a note to moy order saying they found an order if they have?

I suppose it might suggest they found something unusual. I don't know whether they'd add a note for you but I doubt it. If they find flaky components in testing they generally swap them out for another one so there'd be nothing really to tell you.
