Going to do the memtest tonight once I finish work, thanks for the help again.
Thought It was the video card overloading cause I checked the usage which was at 97% but then I lowered the settings to low and still wasn't able to run smooth. FFXIV lags horrifically when I make a move or during a cut scene. Like I run forward and end up taking 3 steps back each time. Then if I tilt the camera I'll get like a scar through the middle occasionally.

When lowering the settings to low it was running at 30%. Forgot to mention.

Just a video of how I'm lagging exactly so you know. During this vid I only had the game and a GPU/CPU monitor open. Aswell as fraps. Monitor was showing 10-20% CPU and 40-80% GPU (sometimes dropped to 0, which i'm guessing is because it's not needed while I'm lagging like that)

When you say its "lagging" like hell What EXACTLY lags. Is it ONLY a program using internet connection or is it ALL programs.

Download and run
http://www.memtest86.com/
(choose your preferred method from the list)

Make sure TRIM is enabled
To enable TRIM, start a Command Prompt window (in Administrator mode, type “CMD” in the Search bar from the Windows Start Menu) and
enter the following command:
fsutil behavior set DisableDeleteNotify 0

Once the value of “0” is set for this parameter, TRIM is enabled.

Check your Cache.
Control panel \ system \ advanced system settings
Advanced Tab \ Performance \ Advanced \ Virtual Memory Change
Select C: and make sure you have system managed size set. Reboot

Remove power from Router for approx 30 seconds and restart

Remove All non needed peripherals like added sound card or usb drives. Add back one by one. Testing each time

Boot to BIOS and check your BIOS Revision number. If it is lower then the BIOS 3201

Download and install
http://dlcdnet.asus.com/pub/ASUS/misc/utils/AISuite_II_V10203_Z68_XPVistaWin7.zip

Download to desktop and extract
http://dlcdnet.asus.com/pub/ASUS/mb/LGA1155/P8Z68-V_GEN3/P8Z68-V-GEN3-ASUS-3201.zip

Run Ai Suite and click Update, and upgrade from file. Point to the file you just extracted to the desktop and let the BIOS update.

Let computer restart and do its bits and pieces.

Once updated go back into Bios and click advanced top right, then advanced again and click the Advanced Tab
Select onboard Device configuration and

PCI express x16_3 slot (black) = Auto
Marvell storage controller = disabled
marvell storage oprom = disabled

Click Back

Select Ai tweaker tab
Memory Frequency = 1600mhz

Save and exit Bios

Go to
http://support.amd.com/us/gpudownload/windows/Pages/auto_detect.aspx
and check your Graphics card drivers

Go to
http://www.intel.com/p/en_US/support/detect
Check your drivers and update as needed.

Restart between EACH update.

Ok, just finished doing all those tips. I'm going to try re-installing the game, maybe thats the reason? Seems to only be lagging when I'm playing that game. So maybe a corrupt file?

Ok well all of above wont hurt eitherway anyway. Did you check you ram with memtest? Results ok?
Did you do an internet speed test make sure you net is actually not at fault?

Memtest86 seemed all ok, no errors popped up after running. Haven't been online in a while. probably test it tonight.
Net speed via Speedtest.net is showing D25mbps U5mbps which is kind of low to what we usually get but I haven't noticed any change in buffering etc. And they're prepping the area for double BB speeds. If 100mbps

Try
Start - type this in Search Box -> COMMAND find at top and RIGHT CLICK - RUN AS ADMIN
Enter this at the prompt - sfc /scannow

Failing that try
Right click C:\ select properties\tools\error checking\check now
Tick auto fix files and scan

Failing that
Enable hot plug on your drive in bios
BIOS-Advanced (top right) advanced again and to the advanced Tab.
Enable Hot plug on 1 drive at a time and see if helps.

See if your SSD firmware can be updated

Have a look at these few things and see how it goes and post back.

Ok, just finished all of this in the last hour. Both scans came back with nothing. BIOS hotplug didn't seem to do anything, what ever it was :p and my SSD is all up to date and 'Intel optimised' Is it a hardware / software problem?

This is what my event viewer says,

1st get rid of Trend for a start. Put on Avast (my fave free atm) or a different anti-virus of choice.

Do you have static IP (same IP address each time you connect to net) or dynamic IP? (different IP each time router is started)
are you using wired or wireless network.

I would as said remove the trend anti-virus, and then set your connection to static IP. Depending on what connection you use you will need to configure it

Control Panel - Network and sharing - change adapter settings.
Right click adapter you use, properties
from list select internet protocol version 4 - properties

Use the following IP =
IP address outside your DHCP settings (you will need to find this in your router config page by logging into your router) DHCP Start Address: DHCP End Address
For EG if your DHCP start address is 192.168.1.1 and your end address is set to 192.168.1.200 then configure your IP address as 192.168.1.201

Subnet mask = 255.255.255.0 (usually automatic)
Default Gateway= 192.168.1.1 (or whatever IP the router uses) **can find by opening cmd prompt and type ipconfig /all

Use the following DNS =
Preferred DNS server:
Alternate DNS server:

Above are your ISP preferred DNS. You can again find these by logging into your router or google the name of your ISP and preferred DNS settings and you should find it easy enough

Then you'll probably have to restart the router and sometimes the PC, too (usually just the router).

If you do the same for wireless, use a different IP address

Ok, now I see what you mean. I don't even know where it came from.. It isn't in my installed files so do I just delete it?
