Yeah, the first thing that caught my attention was the GPU clock speeds - they absolutely should not be that low except maybe when idle, and they ARE causing your performance issues.

I had the same issue as you not too long ago, but I managed to fix it. Please get in touch with PCS and show them this thread (link). They will probably ask you to follow all the other steps first...
Just out of curiosity, does it get as slow when you're not using the Nvidia GPU exclusively? Also, when you run a game, what are your GPU clock speeds?

This morning PCS sent me an OEM fix:
(1) Removing Control Center, (2) rebooting, (3) applying OEM fix, (4) rebooting and (5) reinstalling Control Center seems to have fixed ALL the issues. For future reference:
